Initial Intake & Biometrics
What to Expect After Scheduling Your Clarity Visit:
Once your appointment is confirmed, you’ll receive an email from Healthie, our secure electronic health record platform. This link is good for only three days, so it is best to complete all steps as soon as possible from your home computer. You will also receive an outline that navigates you through our LWC Clarity process.
You’ll be guided through setting up your profile and completing essential forms (best completed on your desktop), including:

LWC Consent Forms
After setting up your profile, we recommend downloading the Healthie app to stay connected with LWC staff for important messages and updates from our team about your future care.
Understanding The LWC “Way of Caring”, Your Story, Nutrition Assessment, and Obtaining Your Biomarkers.
Step 1: Registration and Initial History & Intake
These forms must be completed before orders are sent. Delay in completing such forms could result in your appointment being postponed and or rescheduled.
Step 2: Obtaining your Biomarkers (Bloodwork, Vitals & Body Composition)
All San Diego Residents will come to LWC for their Biometric Visit. California Residents will be sent orders to complete locally at or after their virtual visit, depending on location.
Step3: A Personalized Dietary & Lifestyle Assessment-You will be asked to complete a 3-Day Nutritional & Activity Diary.
These advanced diagnostics & integrative assessments provide deep insights into your hormonal health, metabolic function, and overall wellness. They help construct your LWC Wellness & Vitality Index Profile Report.
Step 2
Initial Clinical Consultation to review your LWC Wellness & Vitality Index Profile-
What to Expect during your Initial Clinical Clarity Visit:
Once all your results are known and reviewed (typically within 1-2 weeks), you’ll return for an in-depth clinical consultation with one of our LWC clinicians
We will take the time to listen further to your concerns, symptoms, and health goals. We discuss your medical history, lifestyle, and any previous treatments you’ve tried. This is your time to share your story and feel truly heard.
We will review your LWC Wellness & Vitality Index Profile Report—a data-driven assessment based on your health intake, nutritional recall, and biomarker results with a LWC Clinician.
From there, we’ll craft together your LWC personalized care plan and empower you with targeted tools and an individualized strategy for:
Hormonal Optimization
Pregnancy Preparation
Sexual Wellness
Metabolic Mastery
Long-Term Vitality
Following this consultation, you may elect to continue your care with Live Well Collective by enrolling in one of our signature, time-limited care pathways.
These pathways are intentionally designed to deliver meaningful, sustainable transformation while aligning precisely with your personal health goals—serving as a refined entry point into comprehensive wellness care. Each pathway is fully individualized and thoughtfully curated, bundling select clinical services and strategic guidance based on your unique needs. Appointments can be booked through our Healthie EMR which contains a patient portal where availability is limited to ensure the highest level of care.

We operate as a cash-based practice, meaning we do not accept insurance. This allows us to provide care without restrictions imposed by insurance companies. We do not commonly provide superbills for reimbursement at this time.

Yes! Many of our services and recommended treatments qualify for Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA). It is necessary however you know what your HSA/FSA does cover. If your services are not deemed medically necessary by your plan you will be required to pay for such services.

Every patient is different, but many start noticing improvements within several weeks of implementing their personalized plan. Long-term, sustainable results require ongoing care and commitment. It is a partnership, and you are a active participant. We are not here to rush the process. Think of it as a marathon, not a sprint.
